Who We Are

"We" are Diagnosify, a company registered in Scotland with our registered office located in Inverness.

"You" are the customer accessing our website (Diagnosify.co.uk) or contacting us by email to order, or have already ordered, our Product(s), activate a test, or create an Account as described below.

Applicability of Terms

These Terms and Conditions along with our Privacy Policy (collectively, the "Terms") govern your use of our website and any telephone or written interactions. In the event of a conflict, these Terms shall prevail over any other terms found elsewhere on our website or in communications between you and us.

Purchasing and Activation

You may purchase Products directly for yourself, creating a personal account ("Account") at Diagnosify, or for others which require activation ("Activation"). Activation involves registering your details in an Account and linking it to a test using an activation code provided in the kit, thus ensuring the laboratory has accurate details for the sample being tested.

User Responsibilities

The sample provider must be the Account holder or have activated the kit.

Accurate personal information including full name, address, gender, and date of birth must be provided upon account creation or kit activation. These details are crucial for sample identification at our partner laboratories.

You must be at least 18 years old to purchase/activate a Product and provide a sample.

Our Products and services are available exclusively within the United Kingdom, including shipping of sample collection kits and processing of samples.

Order Processing and Delivery

Orders are processed within 2 working days (excluding Saturdays, Sundays, and public holidays in the UK). Upon order processing, we will dispatch your sample collection kit by first-class post, expected to arrive within 2 working days, though this is not guaranteed.

If opting for sample collection at one of our partner clinics or by a visiting phlebotomist, ensure the kit is unopened and handled only by qualified personnel. Some partner clinics may already have kits available.

Liability and Medical Advice

Test results and interpretations are provided for informational purposes only and are not a substitute for professional medical advice. Always consult with your GP or another doctor regarding health concerns or abnormal test results.

You are responsible for any decisions to seek or not seek further medical evaluation as recommended based on your test results.

No liability is accepted for non-compliance with test instructions or for any injury resulting from improper use of our kit.

You can reach us by email using the contact details associated with your Diagnosify account. Please include your order confirmation number when contacting us.

We may contact you via email, phone, or postal mail using the contact details provided in your order. This includes, but is not limited to, notifications related to your sample and test results.

Changes and Cancellations

Terms may be amended periodically, applicable to any orders placed at that time.

Contracts are formed upon sending you an Acceptance Confirmation email after payment. Tests are valid for 120 days from the order date.

You may cancel a Contract affected by an Event Outside Our Control if it persists for more than 30 days.

Financial Terms

Payment is accepted via debit or credit card and PayPal as detailed on our checkout page.

Additional charges apply for phlebotomy services or optional extras, with details provided before purchase completion.

Prices may vary, but changes will not affect orders already placed. Prices include VAT applicable in the UK.
